A questionnaire review costs $79 and includes a provider’s evaluation and treatment plan with prescription order.

Medications are billed separately by pharmacy - medical (non-cosmetic) prescriptions sent to retail pharmacies are often covered by insurance.

Follow Up / Refill (3 & 6 month) - $59
Every 3 - 6 months (varies by treatment), a short follow-up questionnaire is required to refill your prescription. This helps us keep your treatment safe and up to date.
