New Questionnaire Review - $49
The initial questionnaire costs $49.99 and includes a provider’s evaluation and treatment plan with prescription order. Each new condition requires a separate questionnaire, each with its own $49.99 fee. Keep in mind our prices might go up as we get busier.

Prescription (Separate Pricing)
Pharmacy prices are NOT INCLUDED - they are set independently and may vary. Medications are billed separately by the pharmacy, not by Minn Skin.

Custom Rx: Most custom prescriptions cost $55 (free shipping).

Follow Up/Refill Questionnaire - $49
Every 6 months, a short follow-up questionnaire is required to renew your prescription. This helps us keep your treatment safe and up to date. Keep in mind our prices might go up as we get busier.
